External medical letters
At Clinic4Sport, we understand that clear and accurate communication is essential for your effective recovery and ongoing support. If you need a medical letter for whatever reason we are happy to support you and facilitate this. We offer external medical letters tailored to your needs, whether it’s for a consultant, GP, school, coach, personal trainer, or strength & conditioning (S&C) coach.
What’s Included in the Letter
Each letter is specific to you and the reason you need it, (i.e tailored for a Doctor, Coach or School etc) and prepared by one of our qualified clinicians
The Letter may include:
Summary of your injury or condition
Details of your current treatment and rehabilitation plan
Recommendations for activity modifications or support required
Information relevant to your performance team (if applicable)
Contact details of your treating clinician for follow-up if required
How It Works
Consent Required: Before we can share your medical information, we will ask for your written permission to include any personal or contact details in the letter.
Flexible Delivery Options: Once the letter is complete, we can either send it directly to you or share it with the external party of your choice, such as a school, coach, or medical professional.
Tailored to Your Needs: Let us know who the letter is for, and we’ll ensure it is written with the right tone and information for that audience.
